Salary Range: $70,000.00 To 75,000.00 Annually

Qualifications 

· LMSW. LMHC, Master’s Degree in a mental health field

· Minimum of two years program experience if not an LMSW/LMCH

· Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile and copier equipment

Essential Duties and Responsibilities

· Provide direct services to clients

· Maintain case records and statistics for program

· Assist the program director in recruiting and training direct service staff

· Administer personnel policies and procedures under the guidance of the program director

· Hold monthly supervision meetings with staff supervised

· Develop regular communication with contract managers for programs supervised; act as liaison with funding source

· Ensure all programs meet contractual goals and reporting requirements

· Monitor program progress and submit all required reports to funding sources, including government and private sources

· Submit internal progress reports on a monthly basis, or more frequently as required

· Plan and develop budgets, and work with Program Director and fiscal staff to ensure that collection of payment from funding source is current

· Monitor fiscal expenses and analyze fiscal reports on a monthly basis

· Prepare proposals for the development, expansion and continuation of programs in coordination with Department Director and the Program Director

· Monitor staff performance, including performing case reviews and structured observations

· Participate in, and encourage staff to become involved in, agency events

· Act as liaison with the community including the area community district

· Perform additional duties as assigned

Program Specific Responsibilities

· Provide timely services to the resident that are consistent with those recommended and described in the HRA-approved application (as applicable), service plan and reviews.

· Develop service plan reviews with the consumer that contain relevant and measurable goals and objectives, including the resident’s expressed goals, based upon strengths, problems, and expressed needs of the resident.

· Complete reviews that contain recommended services and/or service interventions and frequency of each service or service intervention.

· Demonstrate collaboration with the resident in the service plan review process.

· Complete service plan reviews within the required time frames.

· Have signatures of the supervisor and other staff who participated in the development of the service plan reviews.

· Demonstrate in contact notes that services and interventions are related to the goals and objectives of the service plan reviews, and indicate the resident’s progress toward the stated goals and objectives.

· Document changes in recommended care or services in the service plan reviews and other case record documentation.

· Coordinate services with the resident’s treatment and other service providers and make referrals and linkages to additional services for the consumer when necessary.

· Maintain information on all current psychotropic medications prescribed to the resident by qualified health professionals outside of the program, to include: the name and telephone number of the qualified health professional, list of prescribed medications, dosages and dosing intervals of each medication.

· Actively engage the resident who is absent or stops participating in services by conducting appropriate outreach based on the resident’s needs, to maintain continuity of services.

· Document in each contact note that services provided to the resident are consistent with the program’s goals and services, and include: the date of contact; description of the services provided; the name and title of the person providing the services; and the setting of the service.

·Provide the consumer with a minimum of 1 face-to-face contact per month from the supported housing staff, or the number and types of contacts as stipulated in the provider’s contract/agreement. For Supported Housing Programs with a Casement Enhancement the resident receives more than one (at least two) face-to-face contacts per month. For the supported housing resident with whom the provider had no face-to-face contact in the month, there should be documentation of at least one attempted face-to-face contact.

Skills/Abilities

· Strong writing and oral communication skills

· Strong presentation skills and ability to represent the agency at functions and meetings

· Ability to collect and analyze client demographics and outcome

· Excellent organizational, teambuilding and leadership skills

· Ability to use a computer for prolonged periods

· Ability to occasionally lift and/or move up to 10 pounds

· Ability to stand, walk, or sit for long periods of time

· Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork

· Ability to bend and retrieve objects and/or documents

· Ability to travel in the boroughs of New York City and its adjacent counties via public transportation

· Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work
