Job Summary:
WHEDco seeks a Resident Case Manager/Community Coordinator to join its Family Support Services Department. WHEDco’s Family Support Services team links residents of WHEDco’s buildings and Bronx community members to vital resources, including benefits counseling, mental health counseling, school partnerships, an emergency food pantry, and other essential services. The Resident Case Manager/Community Coordinator will provide individual case management, benefits access, mental health, and other services, as well as external referrals to community partners for services beyond WHEDco’s expertise and capacity, to residents of WHEDco’s affordable housing buildings. The Resident Case Manager/Community Coordinator will work collaboratively with WHEDco’s housing and property management teams to provide services to residents and will demonstrate appropriate goal setting and effective practice skills in a collaborative model with WHEDco’s on-site programs. The primary population to be served includes building and community residents at two of WHEDco’s affordable housing developments — Urban Horizons and Intervale Green – located in the South Bronx.
Specifically, Resident Case Manager/Community Coordinator will manage a caseload, providing individual case management services; organize resident activities; and engage in advocacy, resource and referral services to a diverse population. A central focus of this role will be to work with residents to ensure that they attain public benefits, housing assistance, legal assistance, employment/vocational training, mental health services and other critical resources as needed. This can be done in an office setting or in individual resident apartments.
Responsibilities:

Organize, facilitate, and manage advocacy and case management services for building residents, including households that exited the shelter system and/or that are at risk for homelessness

Assess and manage the support needed with individuals and households who may require intervention in the following areas: elder care, substance abuse, mental health, domestic violence, crisis intervention, public benefits advocacy, housing, legal services, among others Provide internal and external referrals and monitor outcomes

With Resident Engagement Manager, assess the need for and coordinate organized tenant activities such as: workshops, support groups, educational and employment events, activities for children, etc.

Along with Resident Engagement Manager, Housing Development and Property management teams, schedule and facilitate regular Resident meetings.

Maintain complete, accurate, and timely client records and case notes into The Data Space on Plan Street, WHEDco’s Data Base.

Attend meetings that include but are not limited to: team meetings, case conferences, interdisciplinary meetings and scheduled supervisory meetings.

Participate in in-house and external staff development and training activities.

Perform other related duties as may be requested by the VP/Director

Qualifications
Qualifications:
- Bilingual English/Spanish required
- Bachelor’s Degree required
- Experience working with people in a permanent residential housing setting
- Minimum of 1-2 years’ related experience preferred
- Motivated, creative and resourceful self-starter who takes initiative
- Ability to move easily and effectively between working alone independently and working collaboratively with a team of co-professionals
- Available for events and/or meetings some evenings
- Knowledge of case management, advocacy, family/youth services and childcare – preferably in a community environment
- Compassionate demeanor with an understanding and appreciation of professional boundaries
- Basic computer skills required (Microsoft Office)
