Primary Function:

The Resident Care Coordinator’s (Social Worker’s) primary responsibility is to provide both ongoing case management services to individuals and households with adults facing urgent and crisis needs, as well as assessing households for additional services and needs. The Resident Care Coordinator (Social Worker) will be responsible for assisting households in completing internal and external referral processes, applications for emergency cash grant assistance if needed, and facilitating follow through and enrollment in other necessary supports through partner agencies and benefit and entitlements through city, state, and federal programs. Additionally, the Resident Care Coordinator (Social Worker) will be a professional support to the individuals and households to assist in overcoming barriers that may present in accessing or completing follow through on required processes and protocols for successfully accessing and enrolling in needed services. Populations may include individuals and households with members who have barriers to accessing services, including but not limited to: severe and persistent mental illness, history or current homelessness or unstable housing, adults with limited social and family support, and physical and developmental diagnoses/disabilities.

Duties and Responsibilities:

Provide direct case work services to individuals and households with adults.  This includes but is not limited to client assessment, information and referral, assistance with applying and recertifying for benefits and entitlements and on-going supportive counseling and case management services.

Incorporate clinical skills and understanding in communications with individuals and households at all times.

Conducting home visits to ensure clients are not causing not living in unstable housing conditions due to mental illness.

Support clients who may have hoarding issues by making referrals to: Adult Protective Services.

Support to clients who may have mental health challenges by making referrals to: ACT Team.

Create, update, and monitor progress of mutually created goals and goal plan between client and Resident Care Coordinator (Social Worker) to manage progress of case management work occurring between Resident Care Coordinator (Social Worker) and the client.

Conduct financial assessments of client’s income and earnings.

Determine client’s eligibility for assistance.

Assist clients in completing recertification documents and gathering necessary documentation, and where appropriate.

Assist clients in completing internal and external referrals and facilitating follow through and enrollment in other necessary supports through partner agencies and benefit and entitlements through city, state, and federal programs.

Adhere to established agency protocols for referrals to internal programs, including benefits and entitlements referrals.

Complete documentation for successful linkage to external resources available.

Complete all applications and maintain appropriate records utilizing the agency’s Salesforce data base and YARDI.

Complete all case notes within 1 week.

Provide all requested information and documentation as required by organization. This may include but is not limited to monthly reporting, client vignettes, statistical data on clients served, and an analysis of the program’s strengths and weaknesses.

Other tasks as needed by the department and/or organization.

Case records in Sales Force are accurate and up-to date.

Skill and Education:

MSW required; education plus minimum 2 years relevant experience required

Computer literate with working knowledge of Word and other software programs

Language Skills: Bilingual Spanish is required
