Title: Program Manager 

Unit: Housing (MOC-J Emergency Transitional Housing)  

Salary Range: $38.36 – 41.20 per hour [Approximately $70,000-75,000 annually] 

Status: Full Time; Regular; Non-Exempt 

Location: 1171 Washington Avenue, Bronx, NY 10456  

Days/Hours: Monday – Friday, 9am – 5pm 


Position Summary: 

To perform this job successfully, an individual must be able to carry out each essential duty satisfactorily. A Program Manager oversees multiple related projects, ensuring they align with organizational goals and deliver value through effective coordination and strategic planning. Under the supervision of the Senior Director, the Program Manager is responsible for ensuring the delivery of case management services within the permanent supportive housing program. 


Mission, and Fit: 

The candidate must have a history of working with a population who have been diagnosed with serious mental illness (SMI) and co-occurring disorders. In addition, the person must be able interact and work with a persons impacted by the legal system and have a history of homelessness. Demonstrated ability to work in a team environment. 


Essential Duties and Responsibilities: 

  • Oversee day-to-day operations of Senior Case Managers, Case Managers, and housing specialist this includes individual, and group supervision. 

  • Develop, implement, and maintain strategic operational systems to strengthen program reporting and evaluation. 

  • Review participants chart to ensure accuracy, quality, and detailed documentation. 

  • Research, evaluate, and maintain linkage agreements with external service providers. 

  • Act as a liaison between Fortune’s subcontractor, Fountain House, and other external providers to ensure coordinated services. 

  • Lead weekly community meetings and facilitate additional groups as required. 

  • Build and maintain a comprehensive network of referral agencies offering services such as mental health, primary health, dental care, substance abuse treatment, education, employment, family services, alternatives to incarceration, housing, and LGBTQ+ counseling. 

  • Prepare and submit weekly and monthly reports to the Senior Director in a timely manner. 

  • Assist the Senior Director with hiring, training, and onboarding new employees. 

  • Provide 24/7 on-call support for emergency situations. 

  • Perform other related duties as assigned. 

Qualifications

Qualifications: 

•HS diploma: some college preferred. 

•At least two years’ experience dealing with HIV/AIDS, substance abuse and/or criminal justice issues 

•Experience of working with clients, at-risk youth and other disenfranchised populations (clients and people in recovery are encouraged to apply) 

•Possess strong follow-up, organizational and leadership skills 

•Possess strong verbal and writing skills 

•Experience using Microsoft Word computer applications 

•Able to encourage disenfranchised populations to seek and obtain the social services needed 

•Possess valid driver’s license and proven driving ability as demonstrated by previous driving record 


Travel Requirements:  

This position requires weekly on-site availability and travel to various Fortune housing and program locations as required.
