Description

Position Summary:

The Health Home Enrollment Specialist (HHES) will be responsible for engaging and enrolling patients for the Health Home program as well as assisting the Health Home Case Managers on site and in the field under the direction of the Health Home supervisors and Director of Care Management. The HHES will act as the first patient point of contact for HH eligible patients with the aim of providing outreach calls, point of care visits, and home visits to engage patients and verify Health Home eligibility. The HHES will complete the consenting part of the enrollment process and complete documentation as required. The HHES will provide care coordination support to the Health Home Case Managers assisting with patient follow ups, home visits, and diligent search activities.
 
Minimum Qualifications:
  • Highschool/GED required. 
  • 2-3 years of direct experience with community outreach or communications in a health care setting.
  • Ability to manage multiple projects, deadlines, and multi-task in a deadline-heavy department.
  • Team-player who thrives in a collaborative environment.
  • Excellent interpersonal skills.
  • Bilingual Spanish/English is required
Key Duties:
  • Screens patients for Health Home (HH) eligibility and offers Health Home services to assigned or internally referred patients. For assigned patients, assure opt out steps are completed for patients who are not interested in services.
  • Begins intake process for patients who are interested in enrolling in the Health Home program; completing and obtaining required Health Home Consent forms and then collaborating with assigned Health Home Case Manager for completion of Health Home enrollment.
  • Aids the Health Home Case Managers by providing patient follow ups, home visits, and diligent search activities as assigned by Health Home Supervisors or Director of Care Management
  • Completes all required documentation and submits timely entry of contacts (within 24 hrs) in tracking system as required (i.e., Excel, ECW, Relevant Health Home, and others as required).
  • Attends staff meetings internally and externally as assigned.
  • Assures patient confidentiality by following all policies and procedures related to disclosure of protected health information.

Work Environment Type, Location, and Schedule:

Intermittent Hybrid, Bronx, NY, Monday – Friday 8am to 5pm or 8:30am-5:30pm.

Starting Compensation: 

Minimum: $44,000 annually; Maximum: $44,000 annually
