Community Health Worker

 

The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers.  In partnership with the NYC Health Department, FPHNYC incubates innovative public health initiatives implemented by NYC Health Department to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

 

POSITION OVERVIEW

The NYC Health Department is seeking a Community Health Worker to provide e-Health Hub support for the community at the Bronx Neighborhood Health location. Currently, many of the NYC Health Department services are available physically or online at distant locations and this can easily result in confusion among the community due to various reasons such as internet access, language barriers, literacy deficiencies, and/or unfamiliarity with navigating online forms. The Community Health Worker will assist the community to access these services through the e-Health Hub, which aims to increase accessibility of NYC Health Department services in one centered location at the Bronx Neighborhood Health.

 

RESPONSIBILITIES

  • Assist with planning and executing health awareness and promotion activities in diverse settings (some evening and weekend work required).
  • Engage and provide health education and information to community partners and residents.
  • Connect individuals to appropriate referrals and NYC Health Department services, mainly through the e-Health Hub.
  • Conduct outreach to community residents, organizations, businesses, and other partners to help advertise the e-Health Hub.
  • Present to key stakeholders, community partners and residents the services that the bureau of the Bronx Neighborhood Health provides via presentations and one-on-one meetings.
  • Serve as a liaison between the NYC Health Department and other providers to optimize coordination of care for the community.
  • Assess for health-related social needs and refer as needed to other programs within the NYC Health Department.
  • Collect and summarize data to monitor individual needs, individual assistance, and outcomes.
  • Take part in public health detailing initiatives in the South Bronx when needed.
  • Performs other duties as assigned.

 

QUALIFICATIONS

  • High school diploma or GED
  • Associate degree or bachelor’s degree in community health or related field preferred.
  • Bilingual (English/Spanish or English/French) is required.
  • Strong relationship builder with excellent interpersonal and communication skills.
  • Experience providing coaching, mentoring, adult learning, counseling, outreach, workshop facilitation, and/or group/community presentations preferred.
  • Experience working with marginalized populations preferred.
  • Healthcare related experience is a plus.

 

SALARY AND BENEFITS

9:00 am– 5:00 pm

Monday – Friday

Fully in office

This position is grant-funded – Grant ends 6/30/2026
