RESPONSIBILITIES:
- Screen clients for service needs and connect clients to services
- Screen clients for SNAP benefit eligibility
- Communicate with other BronxWorks programs and staff
- Complete referrals to BronxWorks services and programs
- Complete program enrollments
- Work collaboratively with Community Health Programs team
- Perform program outreach
- Work closely with Program Coordinator, Activity Specialist, and program staff to meet program targets
- Assist with program set up and break down
- Provide Program Coordinator with weekly reports
QUALIFICATIONS:
- Bachelor’s degree required ·
- One year of work experience preferred
- Bilingual in English and Spanish required
- Experience with SNAP/EBT benefit programs preferred, working with underserved communities a plus
- Experience working with families a plus
- Highly motivated and positive outgoing demeanor
- Proficiency in modern business communications including, personal computers, laptops, cell phones, tablets, iPad, electronic email, voicemail, facsimile and copier equipment
- Must have Saturday availability
PROGRAM SPECIFIC RESPONSIBILITIES:
- Client engagement and case management tasks for Saturday Wellness Program
- Conduct screening for SNAP/EBT benefit eligibility and one-to-one service for benefit related challenges
- Program outreach and recruitment
- Attend program trainings
- Ensure program targets are met
- Complete funder progress reports with assistance from Program Coordinator
- Support program evaluation efforts, track progress, maintain detailed documentation of program cases, and enter data into database
- Complete timely case notes and complete follow up with clients as needed
- Maintain consistent communication with Program Coordinator on program updates, progress, and case management
- Work closely with program staff to identify service needs
- Assist where needed with program facilitation, administrative duties, and community outreach engagement
PHSYICAL REQUIREMENTS
- Ability to use a computer for prolonged periods.
- Ability to occasionally lift and/or move up to 10 pounds.
- Ability to stand, walk, or sit for long periods of time.
- Ability to bend and retrieve objects and/or documents.
- Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
