Salary Range: $64,000.00 To $70,743.00 Annually

RESPONSIBILITIES

  • Communicate effectively with clients, direct reporting staff, peers, supervisors, and funders regarding best program practices.
  •  Demonstrate a strong understanding of contract requirements related to case management and actively share this knowledge with staff to ensure compliance and consistency.
  • Ensure that all client and program files meet funder standards and adhere to BronxWorks’s internal Quality Assurance protocols.
  • Review and analyze program and demographic data to identify trends and inform improvements to service delivery.
  • Provide supervision, guidance, motivation, and performance coaching to direct reporting staff to support professional growth and excellence.
  • Strategically plan and coordinate program activities to achieve contract goals and meet or exceed performance targets.
  • Foster collaboration and enhance team cohesion among staff and peers to improve overall program functioning.
  • Exercise sound judgment and make informed decisions within designated authority levels, following established policies and procedures.
  • Partner with direct reports to strengthen performance through feedback, training, and additional support mechanisms.
  • Navigate and resolve resistance to change among clients, staff, and supervisory or funding bodies with professionalism and sensitivity.
  • Assist with resolving escalated client issues, service coordination challenges, and crisis situations.
  • Implement disciplinary actions as necessary, maintaining a constructive and solutions-focused approach.
  • Support client recruitment efforts by identifying and engaging individuals who meet program eligibility criteria.
  • Collaborate with BronxWorks management to address programmatic and staffing concerns as they arise.
  • Effectively manage personal workload and ensure program operations are efficiently aligned with staff capacity and time.
  • Conduct weekly case conferences to assess client progress and provide staff with necessary guidance and oversight.
  • Audit client documentation regularly for accuracy, completeness, and adherence to program standards.
  • Participate in internal administrative and team meetings to contribute to program planning and improvement.
  • Submit timely and accurate reports to BronxWorks management and/or funders, as required on a weekly, monthly, quarterly, or annual basis.
  • Conduct community outreach to increase program visibility and recruit potential clients.
  • Conduct telephone screenings to assess client eligibility and schedule intake appointments as appropriate.
  • May also assume direct service or program delivery responsibilities, as needed.
  • Responsibilities may evolve or expand over time in response to program needs.
  • Collaborate with internal departments and external partners to support client needs and program objectives.
  • Participate in hiring, disciplinary actions, and performance evaluations for case management staff.
  • Provide training, onboarding, and ongoing professional development to staff.

QUALIFICATIONS

  • LMSW required plus three to five years of experience providing case management services
  • Prior supervisory experience working in a social service agency preferred
  • Must be SIFI Certified
  • Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile and copier equipment

PHYSICAL REQUIREMENTS

 

  • Ability to use a computer for prolonged periods.
  • Ability to occasionally lift and/or move up to 10 pounds.
  • Ability to stand, walk, or sit for long periods of time.
  • Ability to bend and retrieve objects and/or documents.
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
