Senior Case Manager

POSITION OVERVIEW
The Food and Nutrition Program Director (FNPD) under supervision of Director of HIV programs, will oversee day-to-day operations of program. The FNPD will provide weekly supervision to program staff, promote competency and ensure staff receive support needed. Responsible for the development and coordination of staff training in accordance with Food and Nutrition scope. Will collaborate with Care Coordination Redesign Program, Ryan White Part A and other programs to recruit targeted population into Food and Nutrition Program.
KEY ESSENTIAL FUNCTONS
- Responsible for program enrollments (Intake).
- Coordinate and oversee the implementation of Nutritional Assessment and conducts reassessments with clients every six months.
- Distribute bi-weekly food vouchers to program participants based on program compliance.
- Review all eligibility documentation to ensure clients meet eligibility standards for program.
- Coordinate outreach activities to recruit targeted population.
- Conduct random Q/A review of client charts on monthly basis.
- Implement data tracking system for eSHARE reporting of program deliverables. Promotes data process for entering program services in eSHARE and ensures program data tracking reporting is completed in timely manner for submission.
- Collaborate with Care Coordination Redesign program to service mutual clients.
- Oversee Outreach staff with client accompaniments and case findings.
- Attend all RWPA scheduled provider meetings.
- Develop weekly Nutritional Group schedule.
- Ensure all required trainings for grant are completed by program staff.
- Conduct outreach and engagement via phone, electronic methods, letters and or field work to client/collateral/provider to engage clients or strengthen connectivity. Sets up linkages with internal and external resources.
- Maintain consistent supervision with Food and Nutrition staff members.
- Monitor and reviews monthly tracking sheets prior to data submission.
- Ensure the completion and data entry of Primary Care Status Measure via eSHARE.
- Promote Food and Nutrition Program mission, goals and desired outcomes.
- Responsible for Public Health Solutions and Department of Health reporting and submission of monthly electronic program narrative report.
- Demonstrate the ability to clearly articulate, verbally and in writing, the aims and goals of the department to potential patients, community members and staff.
- Accesse and responds, per agency guidelines, to client complaints of grievances as per Policy and Procedures.
- Develop Quality Management Plan utilizing DOH guidelines.
- Provide culturally appropriate health education on topics related to chronic disease prevention, physical activity and nutrition, and cultural competence.
- Conduct noteworthy client participation events, Change Advisory Board and client survey.
- Follow established policies and procedures.
REQUIREMENTS
- HS Diploma/GED.
- Baccalaureate in Social Work or related field preferred.
- Bilingual (English and Spanish) preferred.
- Six years case management experience with HS Diploma/GED.
- Four years case management experience preferred (With Associate degree).
- Two years case management experience preferred (With Baccalaureate degree).
- BASIC skill level of EHR/EMR.
- Standard Microsoft Office, Outlook, Excel, and Internet.
