BronxWorks is hiring six (6) Case Manager for the Bronx Point Program.

RESPONSIBILITIES 

  • Managing a caseload of 14-18 individuals and families with chronic homelessness history
  • Complete assessments and service plans and conduct reviews in collaboration with clients.
  • Service plans contain relevant and measurable goals and objectives, including the client’s expressed goals, based upon strengths, problems, and needs. 
  • Provide timely services to clients that are consistent with those recommended and described in service plans and reviews.
  •  Document and maintain updated information about active medications and medical diagnoses, service providers, emergency contacts, and entitlements for all clients. 
  • Document progress notes for all encounters and services provided to clients, and meet monthly reporting requirements including timely documentation and engagement with full caseload ·
  • Conduct at least 1 face-to-face contact per month per client or more if required by service plan, conduct at least one home visit every month and one formal home visit every three months
  • Assess ongoing eligibility for various entitlements and resources for caseload and complete applications in collaboration with clients 
  • Assist clients with Representative Payee services including budgeting and financial education 
  • Complete Medication Monitoring Training within 3-6 months of hire and assist clients with Medication Monitoring on a daily, weekly, or as needed basis 
  • Coordinate and connect clients to variety of medical, mental health and behavioral health physicians and services, and make referrals to additional services when necessary (ACT, IMT, AOT, Detox, rehabilitation programs, Home Care Services, etc.)
  •  Coordinate clinical services for clients including discharge planning and hospitalizations in collaboration with supervisors 
  • Conduct field work with clients to clinics, hospitals, service providers, and entitlement offices when needed 
  • Conduct regular case conferences with clients and service providers as needed 
  • Maintain familiarity with agency and city resources available to clients 
  • Participate in regular meetings, trainings and weekly supervision with supervisor and other support staff 
  • Actively engage clients who are absent or stop participating in services by conducting appropriate outreach to maintain continuity of services.
  • Perform additional duties as assigned

 QUALIFICATIONS 

  • Bachelor’s degree in social work or another related field of study.
  • One-year of case management or relevant work experience preferred, particularly experience with the homeless population
  • Engaging personality and detailed orientated skills are essential. 
  • Ability to operate in an active environment, manage crises, time management, strong oral and written communication, and organizational skills are essential
  • Proficiency in modern business communications including Microsoft Office suite, personal computers, electronic mail, voicemail, facsimile and copier equipment

 PHYSICAL REQUIREMENTS 

  •  Ability to use a computer for prolonged periods 
  • Ability to occasionally lift and/or move up to 10 pounds 
  • Ability to stand, walk, or sit for long periods of time 
  • Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork 
  • Ability to bend and retrieve objects and/or documents 
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation  
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work 
  • ‘TB Test required with the first 120 days of employment’  for Shelters and Homeless Outreach programs
