DIVISION OVERVIEW

The Center for Health Equity and Community Wellness (CHECW) seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW’s work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers. CHECW is a division of the NYC Department of Health and Mental Hygiene, and this position will be placed at the Health Department through a partnership with FPHNYC.

PROGRAM OVERVIEW

The NYC Public Health Corps (PHC) is a citywide investment in and commitment to the public health workforce and just recovery from COVID-19 with and for communities that have been disproportionately impacted. Following several years of COVID-19 specific work, the next phase of PHC aims to focus on chronic disease prevention and management through decreasing inequities in health and social outcomes for residents of public housing. Program aims include:

  • Reduce the inequities in modifiable risks for chronic disease among public housing residents and residents in Bureau of Neighborhood Health catchment areas
  • Prevent the unfair impact of public health emergencies including COVID-19 and other respiratory viruses on public housing residents and residents in Bureau of Neighborhood Health catchment areas (Harlem, South Bronx, and Central Brooklyn) by promoting vaccination and other preventive measures
  • Screen for social needs and connect residents to services
  • Screen for mental health needs and facilitate connection to mental health services
  • Demonstrate the impact of community health workers on addressing health inequities to inform Medicaid policy and priorities

The Health Advocacy Partners Program, a component of PHC, will drive equitable pandemic recovery in priority public housing communities that have experienced longstanding health inequities. The program will fund community-based organizations to build up and utilize a workforce of community health workers (CHWs) to deeply engage with targeted NYCHA neighborhoods in preventing and managing chronic conditions like asthma, hypertension and diabetes. The program provides:

  • One-on-one health coaching sessions
  • Individual and group-level educational workshops
  • Assistance in acquiring and navigating health insurance
  • Referrals to clinical and social support services in the community

The program model builds on more than a decade of work and program data demonstrating impact and positive outcomes in several communities in Harlem.

POSITION OVERVIEW

The Senior Program Manager for Health Advocacy Partners (HAP) will oversee development and implementation of the Health Advocacy Program in Bronx, Brooklyn, and Queens public housing developments. They will be responsible for overseeing a competitive process to bring partners on board; overseeing program expansion across multiple sites, including partner onboarding and training; and ensuring grant deliverables and targets are met, tracked, and reported. There may also be the opportunity to supervise a program coordinator and related team members following program expansion. They will be responsible for guiding, supporting, and monitoring contracted community-based partners’ delivery of health coaching services, social support referrals, and wellness activities geared towards meeting program goals. The candidate should be able to demonstrate experience and success in implementing technical assistance, capacity building and engagement strategies with a racial equity lens to community-based organizations to improve health outcomes. The Senior Program Manager will assist strategic planning of community and stakeholder engagement. The Senior Program Manager will report into the Office of the Chief Medical Officer and Deputy Commissioner.

Given the nature of the ever-evolving landscape, some evening and weekend work may be required but compensated. Occasional travel to partners in Brooklyn, the Bronx, and Queens may be needed.

RESPONSIBILITIES

  • Oversee expansion of the Health Advocacy Programs, including partner solicitation and selection; contract development and review; partner onboarding; and program launch, including reporting
  • Oversee management of deliverables for approximately three community- and faith-based organizations working on NYCHA properties.
  • Oversee partner engagement and capacity building strategies and ensure alignment with overall program goals.
  • Support contract development, amendments and compliance in partnership with contracts team members
  • Provide technical assistance on contracting, invoicing, and other administrative processes to partner organizations.
  • Track and manage all grant contracts, invoices, and other administrative components to ensure compliance.
  • Collaborate with the Director of Health and Housing on program planning, NYCHA partnership, and sustainability after the grant period.

  • Support on data collection and communications to stakeholders.
  • Support partner reporting needs.
  • Support partners in navigating NYCHA resources, protocols, and policies.

QUALIFICATIONS

·         Master’s degree in Public Health, Social Work, Social Services, Public Administration, Human Services, or related field highly desirable; Bachelor’s degree with experience in these areas acceptable

·         Experience managing contracted partners

·         Direct supervisory or managerial experience

·         Strong relationship builder with excellent interpersonal and communication skills

·         Ability to be work independently, be flexible, and handle changing workload with multiple projects and changing priorities

·         Experience working with highly marginalized populations

·         Ability to operationalize workflows

·         A commitment to collective action and community organizing

·         Experience planning and implementing neighborhood-based programs

·         Experience in healthcare and/or social service administration with content focus in transformation, community/clinical linkages, integration and/or population health

·         Knowledge of the NYCHA community and public housing landscape

·         Understanding of community health workers and the role they play in health inequities

ADDITIONAL INFORMATION

This position is funded until June 30, 2024, with possibility of extension. Additionally, there is a potential for this position to transition to the Health Department and therefore candidates must meet their eligibility requirements, including NYC residency.
