NCS Connect reaches individuals who may be in need of services, but are disconnected from them. NCS Connect has a presence at free meal programs on the Upper East Side of Manhattan five evenings each week to provide guests with information and referrals for shelter and housing, public assistance, counseling, mental health and substance use treatment, and a variety of other services available in the area and the City. Chance to Connect (CTC) is a new program located on the Lower Level of Madison Avenue Presbyterian Church. Guests referred from NCS Connect will be able to come to CTC for follow-up and light case management, as well as for a respite with coffee, snacks, WiFi, activities, toiletries and other essentials.

Description

Position Summary: Provide social work services for NCS Connect’s mobile information, referral, and case management program for low-income, homeless and at-risk households patronizing community meal programs and at Chance to Connect, NCS’s resource center for NCS Connect clients.

Responsibilities:

  • Provide information and referrals for individuals attending meal programs at Upper East Side locations
  • Provide information and referrals at CTC, including follow up for NCS Connect clients.
  • Maintain access to current information on community resources
  • Intake, enrollment, maintain charts and case files
  • Supervise program volunteers
  • Maintain statistics and provide written program reports as necessary
  • Travel to various locations in Manhattan
  • Other duties as assigned

Qualifications:

  • Requires a Bachelor’s degree in Social Work or a related field, Master’s Degree in Social Work preferred.
  • Prior experience in social service environment, particularly working with homeless/formerly homeless, mentally ill and/or chemically addicted individuals.
  • Knowledge of community resources preferred.
  • Strong time management skills.
  • Knowledge of Windows, Microsoft Word, and Excel.
  • Knowledge of AWARDS strongly preferred.
  • Strong English language written and verbal communication skills; Bilingual in Spanish is a plus.

Salary: $42-50k annually
