Description

Program Specific Responsibilities

  • Participates in all related FTC’s and case practice policies and procedures are being implemented and adhered to
  • Schedules all conferences within the appropriate time envelope or in line with related circumstances
  • Submit internal progress reports on a monthly basis, or more frequently as required.
  • Models and trains staff in conference facilitation skills so that they are strengthened over time
  • Insure that all conferences are substantive and genuinely helpful to the staff and to the clients
  • Reviews and ensures proper documentation in Contact Grids, PROMIS, Connections and offline documentation, including the listing of contacts, progress notes, Family Assessment and Service Plans and Family Team Conferences.
  • Joins or represents the Director on meetings of relevance to the department’s work (ACS, OCFS, COFCCA).
  • Complete training for FTC Facilitation and ACS training requirements
  •  Identify, evaluate and address any and all safety and risk concerns during conferences.
  • Complete timely documentation in agency databases
  • Develop working knowledge of all current FTC standards and policies
  •  Collect statistical data on FTCs, compile and present reports on a monthly basis.
  • Present FTC data and share information at inter and intra agency meetings
  • Monitor outcomes related to identifying performance trends, best practice and areas of improvement
  • familiarize new staff, as needed, with various FTC and program compliance mandates
  • Conduct pre and post FTC briefing of case with case planner and their supervisor.
  •  participate in team meeting, training and program planning
  • Attends trainings to improve skills and strengthen practice.
  • Maintain availability and access for staff to consult when they are in the field.
  • Develop regular communication with related collaterals; act as liaison with funding source (ACS)
  • Facilitate groups as needed.
  • Supports staff program orientation and onboarding
  • Carries cases as needed during staff shortage.
  • Perform additional duties as assigned by manager.

General Responsibilities

  • Collect statistical information from CARES, other required databases to write and complete reports
  • Collect information directly from staff and program supervisors necessary to compile required reports
  • Extract data from CARES and other data sources to identify important trends and problems
  • Present data findings in inter-agency and internal meetings
  • Track Program assets in Asset Panda or another database program
  • Monitor staff remote productivity performance on Teramind.
  • Participate in Program Fiscal Meetings.
  • Prepare weekly reports as required
  • Participate in other agency initiatives as assigned.
  • Oversee the implementation of the ETO system for program

Qualifications

  • Bachelor degree in in social work or another related field of study preferred.
  • Strong oral and written communication, time management and organizational skills are necessary.
  • Proficiency in Microsoft Office suite and other standard business technology is required

Physical Requirements

  • Full vaccination against COVID-19 is required.
  • Ability to use a computer for prolonged periods.
  • Ability to occasionally lift and/or move up to 10 pounds.
  • Ability to stand, walk, or sit for long periods of time.
  • Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork.
  • Ability to bend and retrieve objects and/or documents.
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
